City Safety Manual
Note: In the coming months, the Safety Committee will be working with Committee
staff support to revise the draft City Safety Manual developed by CIS {see
Attachment "B") to meet the needs of all City departments and divisions. The
attached manual contains only one of two parts, and we asl< that all committee
members begin reviewing the manual in preparation for the revision process, which
is expected to begin at the April meeting.

Dear Mr. Runyan:
On Thursday, January 4th, a safety inspector from your Department
performed a safety inspection of the elevator located in Woodburn City Hall.
During the inspection, a city employee attempted to use the elevator, and nearly
walked into the empty shaft before noticing that the elevator car was in the
basement and under inspection. She did not see any warning signs, and the call
button had not been disabled. She was worried that the incident could be
repeated by other employees or the public with harmful results. She contacted
Woodburn Safety Committee staff support to report the incident, and staff
support forwarded the e-mail to the Building Maintenance Division,
management, and the Safety Committee.
Building Maintenance staff and management indicated that they were
aware of the incident, and committed to monitoring the inspection process
more closely during the next inspection. At the Safety Committee meeting held
on January 18th, members unanimously agreed that the City should contact
your Division to request cooperation in ensuring that this incident is noted and
not repeated.
The City appreciates the service that the Department of Consumer &
Business Services provides, and has an excellent working relationship with your
inspector, Jim Powell. The intent of this letter is not to assign blame, but rather

The Elevator Safety Program is in receipt of your letter regarding an inspection that
occurred on January 4, 2001. Thank you for bringing this issue to our attention.
Inspectors are required to take all necessary safety precautions when performing safety
inspections on elevators. On behalf of the Elevator Safety Program I apologize for any
concern this may have caused at your facility.
Inspectors will be reminded of their obligation to provide a safe environment when
inspecting elevators.
